Player and volunteer photographs and video may be taken from time to time and used in FC Columbia training and promotional materials on our website and social media. Member identities shall not be divulged in this process and the identities of players shall be protected in all public forums.
The parent or legal guardian of an FC Columbia player grants FC Columbia permission to use photographs for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. No royalty fee or other compensation shall become payable to the parent/legal guardian or FC Columbia of such use. If a parent or legal guardian opposes such release, please submit the form below.
